Individuals desiring counseling services for themselves, or those in their care, need to complete the\u00a0Counselee Services Intake form<\/a>\u00a0and submit it to us by\u00a0secure email<\/strong> to cfcc1945@gmail.com<\/strong>. If you need assistance in sending a secure email you should reach out to your email provider on the correct process. Sending a secure email encrypts the private health information you’re attaching to your email so no one can open and read it except your intended recipient. Here are links to pages about email encryption for two of the most common email providers: Microsoft Outlook <\/a>and Google Gmail<\/a>. <\/p>\n\n\n\n

The Counselee Services Intake form<\/a> is an Adobe PDF (Portable Document Format) file and a minimum version of Adobe Reader 5.0 is required. If you have don’t have Adobe Reader or you have an older version than 5.0, you can download the most recent version free from Adobe’s website<\/a>. Follow their instructions for downloading and installing the program and then return to this page to download the form.<\/p>\n\n\n\n
